Pretty short and to the point but your right there was no option for decline to disclose.

On second thought, vaxxed and partly vaxxed folks could throw a monkey wrench into the survey (like checking a “decline to disclose”) by checking box #1.
If they didn’t tell anyone they really did get vaxxed, the company couldn’t tell whether or not they actually did.
Similarly for unvaxxed, if they checked #1, and stayed quiet about not getting it, they also could throw a monkey wrench into the survey.
If a company gets 30 to 50% of the replies on #1, what are they going to do, fire everyone?
