Health Plans, including health insurance companies, HMOs, company health plans, and certain government programs that pay for health care, such as Medicare and Medicaid.
Most Health Care Providers—those that conduct certain business electronically, such as electronically billing your health insurance—including most doctors, clinics, hospitals, psychologists, chiropractors, nursing homes, pharmacies, and dentists.
Health Care Clearinghouses—entities that process nonstandard health information they receive from another entity into a standard (i.e., standard electronic format or data content), or vice versa.
Companies that help your doctors get paid for providing health care, including billing companies and companies that process your health care claims
Companies that help administer health plans
People like outside lawyers, accountants, and IT specialists
Companies that store or destroy medical records

The HIPAA laws don’t prevent your employer from asking for a doctor’s excuse if you’ve been off work due to illness. But it does prevent the employer from calling the provider and asking the reason why you’ve been off sick without you giving permission for the employer to have that info.
The job I do falls under HIPAA. I’m not a lawyer, but I have yearly training about HIPAA.
If your spouse was on our health plan and you were covered as a dependent, you could call me and I could talk to you about YOUR coverage. I could not tell you if your spouse was covered or if anyone else is covered on his insurance.
I don’t know how many times I’ve had the following conversation:
Spouse: my doctor says I don’t have insurance.
Me: You do not have coverage on our plan at the current time, correct.
Spouse: well, I’m supposed to be on my spouse’s plan. Are THEY covered?
Me: I can’t tell you that.

It looks like you are able to inquire about basic health coverage & payments.. Does it also allow for questions about specific medical conditions or treatment or medications for the employee?

Well...define ‘you’. Are you talking me in the performance of my job, a employer calling one of these entities with a question, or the patient themselves?
For example, I’ve called an insurance provider when a member calls saying they are having an issue getting a claim paid. Since both I and the insurance provider are ‘entities’ we can talk to each other. So they can tell me things like ‘the colonoscopy claim was submitted as diagnostic and not preventative’.
I started decades ago when my MD told me to take fish oil and CQ10 since I couldn’t take statiins, and then told me to take D3 for joint pain.

Is the employer allowed to ask about specific procedures, medications, conditions....regarding an employee?
I guess a follow-up question would be is an employer allowed to terminate an employee based solely upon medical information (assuming it can be divulged by the medical office)? I suppose it would be on a case-by-case basis. But for example, if an employee did absolutely nothing wrong at work, but through calling the insurance, the employer found out the employee was schizophrenic but taking medication, could the be fired only based on that information?
