Yes a verifiable form of ID is required, because there are records kept, believe it or not. Things such as your address, age, and a photographic image of your face are scanned in, and there is a tracking system in place to follow up on failure to establish immunity (getting COVID-19 Wuhan virus AFTER being vaccinated), and the conspicuous absence of your name on the list of the already-vaccinated will be noted for future reference.
Only for illegal aliens.
This is odd. When I go to the doctor, even the emergency room, I am required to present a photo ID. There’s a sign to that effect right at check-in. However, it is against the law for them to ask if I’m a citizen. A nurse told me Hispanics often give a fake name, which to me means, no ID. I am also required to present a photo ID when receiving medicine. Someone else on the forum was complaining about having to present an ID to receive the Covid vaccine. (As Victor David Hansen has pointed out, the law is only enforced against citizens. It does not apply to illegals.)
As far as I know, the requirement for an ID occurred because a college girl had checked her friend, who had no insurance, into the hospital using her name and her insurance. The girl died and the wrong parents were notified. Apparently, it caused some significant legal action and the ID requirement has sense proliferated. (Much as HIPPA has proliferated to the point of stupidity.)

The forces of evil only complain about an ID requirement when there is a chance it will interfere with their voter fraud.
Fraudulently documented foreigners have more documents than citizens do, many in different names, different addresses.
Citizens have SS#’s and can be tracked down by collection agencies, illegal aliens are not. Even the payment for services doesn’t apply to them.
So, get an unneeded experimental “vaccine” for a “disease” that is 99.85% survivable across all age and demographics and get all your biometrics recorded by the govt.

You are being given a Drug that may have potential negative side effects.
I would think they would want to know who is being given the drug.
If you are given a prescription from your Doctor you have to prove your identity, especially with more restricted classes of Drugs.
On that note, yes I had to show ID when I received the Vaccine.
Did they make you sign a waiver of liability?
