As for Miranda rights, they are only regularly read immediately on TV and in the movies. Suspects can be held for hours without having them read. The only consequences of not reading them would be making any confession inadmissible.
Yes most of the time an officer does loosen their weapon in the holster when approaching a car. It is simply good defensive behavior
My understanding is the Miranda Rights only have to be read to you before you are officially charged, or interrogated about a crime in which you are a suspect.
If arrested, they are usually given to you in writing at the police station. You sign that you have received a copy.

Only required is the person is placed under arrest. Being detained is not officially an arrest.
