Looks like six toes to me. Didn't they call Ann Boleyn a witch for her six fingers (11 all togetha)? Sign of a witch or something. Seems appropriate here.
As for whether she would appear in public in sandals with six toes, I seriously doubt she intended on appearing in public at all here, at least not without her weave. No, this was a private moment to let the piggies breathe.

There was a most unsettling picture of Michelle wearing a dress at a White House jazz night
or one of their many other soirees on our dollar.
There was a very odd bulge in the front of the dress
