Write your own statement. Your statement should include your name and address, your attorney's name and address, and an explanation of the circumstances and details of your complaint. Attach any relevant documents you have and send it by fax to (202) 638-0862, by email to email@example.com, or by mail to:

Filing a complaint against an attorney is a serious matter. Do so only as a last resort when all efforts to work out the problem with your attorney have failed.
File it in Farsi. That would be fun.
As a rule, though, complaints by members of the general public about a judge's official conduct are not going to get much attention. Those sorts of complaints should be directed to your Congressman with a request that he be impeached.
But did the Judge use this simple procedure to report to the bar the prosecutor who violated his discovery Oder and intentionally withheld exculpatory evidence?
