Measles and smallpox do not look similar. There is no way that a descently well trained clinician would confuse the two diseases. Measles can feature Koplik spots which are small white mucosal lesions in the mouth. Smallpox does not have this feature. In addition, the rash of measles is nothing like smallpox. A measles rash is generally flat. The smallpox is a "pox" lesion. They're different and any person with one year of medical school training should recognize the difference.

I'm only supposing measles due to the anecdotal evidence referenced in the first link I provided. Besides a lack of trained medical personnel, it seems there is a translation problem as well in the regions where "smallpox" has been reported.
