Or are they in between shots, still looking for an appointment, or never planning to get a vaccine at all? Might they be in chemotherapy, or otherwise immunocompromised in some way that would prevent them from either getting a vaccine or experiencing its full benefits?

Vaccines are not 100% effective but that's not what she said. There are about 200 deaths total from breakthrough infections compared to 600 a day from COVID and over 3000 a day at the peak. That is what I would call "remarkably effective"/
