Because an ED is not a research degree. Just as nobody calls lawyers “Dr.” even though they have a “Juris Doctorate” degree. An ED is like a JD. Lawyers have the degree but it is not normally considered a research degree. In the snobby world of academia, an ED is considered to be almost a vocational degree or one that the holder does not plan to use in a research institution. It is what school teachers obtain to get a two-step bump on the pay scale.
Jill Biden, like Bill Cosby, has an ED which normally means no dissertation or one with reduced research requirements. Even a Ph.D. from a middle-rank university in a communist country like I have is generally considered a better degree

I dunno. I have a science Ph.D. from Harvard, with a well-written thesis containing an unexpected discovery in addition to the work that was originally expected of me. So I am one of the many non-M.D.s who deserve to be called “Doctor”.
