Loo at the other people in the shot on the right. Two of the three are in short sleeves. Hillary is in a heavy coat and a scarf. A vest would not necessitate a scarf. A back brace would.

Look at the people in the background of the photo you posted. They’re wearing short sleeves, so it’s obviously a warm day. Hillary, however, is wearing a coat, and has a scarf around her neck. She most likely has a back brace under that coat.
