How about “Ma Clinton”? I prefer to use "Mrs. Bill Clinton" because it's a subtle reminder that she is nothing without Bill.

Actually, the correct term is ‘transvestite’ - someone who dresses and acts like the opposite sex.
Transgender is something like ‘el basura’, where the correct article is ‘la’.
