Thats the hard reality and that is why the smart ones leave and cut themselves off if they get out.

I've seen fights like these. One got me a trip to the hospital (nothing serious). I intervened and stopped a fight, and took a few punches in the process. No regrets, though.
Anyway, there is also the shock-and-awe factor. This things usually explode quickly. By the time everything registers to a decent bystander, it could darn well be over.
I know you're not excusing the lack of bystander action here, and neither am I. But not everyone is a coiled spring, ready to jump into action at a moment's notice.
Tough call. Were only one other person observing this or a situation like it, response is not so simple. I would say at the most, active intervention if armed or otherwise capable. At the least, trying to find others and encourage intervention.
