Late 50's or early 60's.
It's actually pretty uncommon these days. More likely to be noticed when it happens however.

I remember that until not too long ago (well within my lifetime), the Washington, DC, Police boasted that they had never taken a cop-killer alive.
I would have liked to think that if the cops hit this poor woman *by mistake* enough times to kill her, they could have hit the criminal they *were* aiming for enough times to make sure he was dead.
