The image below was sent to me by a reader who went for a visit to a physician, and was told that she could not be seen unless she filled out this race and ethnicity questionnaire, and put her name on the form.
She initially refused, but they insisted that it was required or she could not see the doctor. The form does indicate that it is mandatory under Section 4302 of Obamacare.
Section 4302 is explained by HHS as follows:

Section 4302 (Understanding health disparities: data collection and analysis) of the ACA focuses on the standardization, collection, analysis, and reporting of health disparities data. While data alone will not reduce disparities, it can be foundational to our efforts to understand the causes, design effective responses, and evaluate our progress.
Section 4302 requires the Secretary of DHHS to establish data collection standards for race, ethnicity, sex, primary language, and disability status. The law requires that, once established, these data collection standards be used, to the extent practicable, in all national population health surveys. In response to this statutory requirement, this implementation guidance outlines the new minimum data collection standards for race, ethnicity, sex, primary language and disability status for implementation in HHS, along with a description of the data standards development process, the rationale for each data standard, and instructions for their implementation.

Just between us, I am sure it will go no farther, under race and ethnic background I threw in white, black, hispanic, American Indian, and then Eskimo, Samonan, Portuguese, and something else that I can’t spell. In truth I am a Melungeon and nobody knows what the hell we are up of, so prove I aint all of the above.
Since I know my doctor is in essence now a government employee I no longer tell my doc the truth about anything. I treat myself as best I can. Broad spectrum antibiotics and penicillin can be had at Tractor Supply or Rural King without a prescription and you can buy needles also. Stock up on vitamins and supplements as the pharma cos. will probably be the only source in the future and that will require a scrip. Do not purchase aspirin or anything else that you put in your body that is made in any Asian country. Read on the label and if it says distributed by a well known American company you can bet it is made in China. If the Chinese will poison your cat or dog what makes you think you are special?

Transracial and transethnic
Had a shoulder cat scan done at a local hospital today. I’d never been there so they had to collect info.
They wanted me to lay my hand on an palm scanner so they could scan my palm. I asked the purpose and was told it would help ID me the next time I came in.
One word was screaming out in my mind....Belarus, Belarus, Belarus.
I said nope...don’t want to do that. Lady said OK.
Pretty sure I’m on “THE LIST”...if I wasn’t already.
