The image below was sent to me by a reader who went for a visit to a physician, and was told that she could not be seen unless she filled out this race and ethnicity questionnaire, and put her name on the form.
She initially refused, but they insisted that it was required or she could not see the doctor. The form does indicate that it is mandatory under Section 4302 of Obamacare.
Section 4302 is explained by HHS as follows:

Section 4302 (Understanding health disparities: data collection and analysis) of the ACA focuses on the standardization, collection, analysis, and reporting of health disparities data. While data alone will not reduce disparities, it can be foundational to our efforts to understand the causes, design effective responses, and evaluate our progress.
Section 4302 requires the Secretary of DHHS to establish data collection standards for race, ethnicity, sex, primary language, and disability status. The law requires that, once established, these data collection standards be used, to the extent practicable, in all national population health surveys. In response to this statutory requirement, this implementation guidance outlines the new minimum data collection standards for race, ethnicity, sex, primary language and disability status for implementation in HHS, along with a description of the data standards development process, the rationale for each data standard, and instructions for their implementation.

That's not a "national population health survey". It's a doctor's office questionnaire, and there is no possible way my answers to those questions could in any way effect my treatment. I'd conclude that the doctor is a moron, a danger to my health, and walk out just on that basis.
Well, they need a way to know who to deny care to, and who to grant it to, and who to charge to pay for those who receive care.

“Mixed” isn’t on the list, but it also doesn’t say “Check all that apply.” Granted, it doesn’t say to check only one, either.
I’m calling BS on this, those instructions would never pass muster at any business I have ever worked at. And that list doesn’t even make sense.
If they really cared about things like the differences race makes in disease susceptibility, etc, they wouldn’t crucify people like Jason Richwine for doing genetic research in such matters.
Just lie.

The obvious solution to this is to make up stuff. I, personally, always identify myself as an Andalusian “Other Pacific Islander,” which I think nicely splits the difference with the idiocy of the questions. Only once has anyone blinked an eye. She didn’t know what Andalusian meant. I explained about the Iberian peninsula and her eyes glazed over and she let it drop. I recommend everyone decide, creatively, on their race and ethnicity and stick with it. Worked for Elizabeth Warren.

No, but I have filled out forms from them, and these instructions wouldn’t pass muster there either. I think this is a fake.

Aside from the Nøzi monstrosity at its root, there’s something very odd about that ‘form’ .. disproportionate preponderance of southeast Asian, Micronesian, Pacific island, etc.
So I blanked out the form and didn't send it in.
At some point we're all going to probably be forced to fill out such forms.
Just because a bunch of stupid celebrities decided to OD on prescription drugs doesn't mean that my health records should be open to the DEA, etc. just in case I'm a druggie too.
I would check off all of them and when they questioned me I would answer that I am of the race of man, one of God’s children. Then I would smile and say FOAD.
I do believe someone’s having us on.
Whether accidentally or intentionally is still in question.
