My "Certificate of Life Birth" from about the same time (admittedly from Illinois) was a hand-filled form with things like: Mother's residence (and how long she lived there), Parents names, ages, races, birth places, occupation (only for the father, from those Ozzie and Harriot days), number of other children (living and dead), parent's signatures, doctor's signature address and phone number.

A few years ago, I thought I had lost the original (I had just misplaced it), so I sent for a replacement, and the replacement was a notarized photocopy of the original.

Yes, I have that information on mine. Mine is called “Certificate of Birth” which I assumed meant a legitimate birth certificate. And you’re right — you don’t get an original — it’s just a stamped, sealed copy.

Anyway, I don’t know about passports, but for the business I needed the certificate for, mine was accepted.

Mine from Chicago was a pre-Xerox photocopy. It looks like it was filled out with quill pens.
