Nope ... I’m not going to answer any survey that wants my email address.
They send your survey results directly to your elected Congressmen/women. It’s quick and easy, and is required by the Sneators and Representatives.

I filled it out and sent it but always use a fake name, address, and email. That way I am not spammed by their offices yet they still get the information.

Listen, I need your bank number and pin number for a govt. survey. Yeah, that’s the ticket, a govt. survey.
enter a phony name and e-mail address. As simple as that.
I did it. The issue is big enough to use a secondary email address, not my primary.
