I have traveled the "Blue Line" many times. This incident occurred where there is a bend in the tunnel. Where the passengers were coming up directly correlates to this point. The line goes from a due west heading to a northwest heading. If a brake locked up at this point, I would think it could have been enough to rattle the last car loose. It shakes pretty good going through there to begin with.
Chicago has been receiving ample practice keeping incident scenes secure and dealing quickly and efficiently with related casualties.
