I have not read the entire article - but how does "spiritual formation" differ from discipleship?

Not much, I wouldn't think, depending on one's definitions and terminology. I guess the subtle difference would be that discipleship implies following Christ, and trying as best as possible to do as He did, whereas spiritual formation would be the extension of that, where we strive to have the same personality, the same spiritual countenance as Christ.

Neither have I (yet), but I'd bet the difference is primarely in marketing. Books with "discipleship" in the title probably don't sell as well.
