LA botox doctor learns fate for $45M Medicare scheme that funded luxury vacations
NY Post ^ | 10/30/2026 | Daniel Farr

A crooked California doctor who ripped off Medicare for more than $45 million to bankroll luxury vacations, expensive cars, and a jaw-dropping $12,000 antique crossbow is finally facing serious prison time.

Violetta Mailyan, 45, of Glendale, was sentenced to 15 years behind bars after a jury found her guilty of running an enormous fraud operation through her Healthy Way Medical Center clinic, submitting fraudulent claims for Botox treatments that were medically unnecessary, purely cosmetic or never performed at all.

The staggering scam wasn’t just about bogus paperwork. Mailyan billed taxpayers for treatments supposedly administered while her clinic was shuttered — and even while she was thousands of miles away enjoying vacations.

Evidence presented at trial showed more than $19 million in claims involved Botox injections supposedly performed on days when the clinic was closed.

Mailyan also submitted claims while traveling to Mexico, Hawaii, Las Vegas, Pennsylvania and New York.

In one particularly brazen example, she billed Medicare more than $200,000 for treatments supposedly performed on California patients while she was actually soaking up the sun on a beach in Cabo.

Medicare reimburses Botox injections when medically necessary to treat documented chronic migraines.

But prosecutors showed that Mailyan exploited the program by billing for thousands of procedures that never happened, were administered for cosmetic purposes or involved patients without referrals from their primary care physicians for chronic migraine treatment.

And the alleged fraud helped bankroll quite a shopping spree.

$45 million in botox treatment billed to medicare and nobody batted an eye? Not one single question? Nobody asked why we are paying for botox to begin with? No concern at all about where my tax money goes? Nothing?

“ $45 million in botox treatment billed to medicare and nobody batted an eye?”

It is likely that the only people to see these claims were bureaucratic workers whose job was to rubber stamp whatever came across their desk. Not their job to raise questions.

It's a foreigner of some stripe, citizen or not. Another parasite. You can tell by mere inspection. She's not a corn fed American girl. She is some sort of whoreish parasite. These foreigners always have faces that look like plastic masques.

One excellent application of AI is pattern recognition over a large sample size, IF you want AI to look for it.

This fraudster had been scamming Medicare since 2021, about 4 years before AI got rolling. Now AI is playing catch-up and finding these bitches and bastards.

She went on quite a shopping spree using stolen tax dollars:
<><>numerous luxury items
<><>a $12,000 17th-century collectible crossbow
<><>a $3,000 painting.
<><>two Tesla vehicles,
<><>a Cybertruck,
<><>and five pieces of California real estate.

The sentencing judge delivered a blistering assessment of the fraud, describing Mailyan’s MO as “truly breathtaking in scope and shocking in its duration,” and her conduct as “almost unbelievable in its brazenness.”

That’s what absolutely enrages and infuriates me. I worked in maybe ten companies in my career and the bean counters watched EVERYTHING. FORTY YEARS ago the engineering firms I hired would send us voluminous invoices that tracked everything right down to every sheet of paper that was used in a copier.

This lack of the most basic accounting controls at every level of government all across the country is staggering.

Yes. I’ve got lots of personal health data from a wide variety of sources going back decades. I recently started using AI to analyze this data set and the correlations it finds are amazing.

Let me take a wild guess here. She was probably the same “doctor” that decided botox was medically necessary for clients that could afford it anyway but why not let the little people pay for it.

Definitely NOT GUILTY by FR standards...but guilty (based on the article) if I were on the jury for this case.

I suspect that the handful of Trump people in DC are doing just that, which is they’re actually finding stuff. The bigger problem is at DOJ, where its run by a guy named “cash” (just kidding), where it takes a huge amount of work to put together a strong case (although AI can sure help there, too).

Difference between private sector and government ..
Private sector goes out of business without efficient operation. Government gets bigger with waste, fraud & abuse. Just tax more the taxpayers.

The only reason doctors accept Medicare is that it pays all claims promptly. The rates are low, but you get your money immediately. It is basically a computer system - nobody looks at the claims.

Enforcement of fraud is after the fact. Computer programs find patterns that are likely to be fraud, and these are investigated by human investigators. This takes a while.

The most important questioned, seemingly never discussed, is how much can be clawed back? Will her property be impounded?
