A new Department of Homeland Security inspector general report concludes that the U.S. Secret Service failed to act on dozens of warning signs before the attempted assassination of President Donald Trump at his July 13, 2024, campaign rally in Butler, Pennsylvania.
The report, released Thursday by the DHS Office of Inspector General, details a cascade of communication failures, security lapses, and planning deficiencies that allowed gunman Thomas Matthew Crooks to climb onto a nearby rooftop and open fire during the rally. Crooks was ultimately killed by a Secret Service sniper.
Among the report’s most striking findings is that Secret Service personnel failed to receive 102 radio transmissions from local law enforcement regarding an increasingly urgent search for a suspicious individual in the area.
Instead, agents assigned to protect Trump reportedly received only five phone calls and three text messages regarding Crooks before the shooting.
“As a result, Secret Service members did not alert President Trump’s protective detail about concerns of a suspicious person,” the inspector general’s report states.
According to investigators, had Trump’s security detail been fully informed about the search for Crooks, the president’s speech likely would have been delayed or he would have been removed from the stage before shots were fired.
The report also found that the Secret Service failed to detect a drone operated by Crooks roughly two hours before the attack. Investigators determined that the agency’s counter-drone system was inoperable because of an equipment malfunction, while the lone operator assigned to the rally lacked sufficient training to restore the system.
As a result, Crooks was able to fly the drone for nearly nine minutes, surveying both the rally grounds and the rooftop he later used to carry out the attack.
Investigators also documented major communication breakdowns between federal and local law enforcement. The Secret Service reportedly never received three separate radio reports indicating that Crooks had climbed onto a rooftop carrying a rifle.
The inspector general further concluded that classified intelligence concerning a long-range threat against Trump was not shared with the Secret Service’s Pittsburgh field office or the agents responsible for planning security for the Butler rally.
The report states that if the intelligence had been properly disseminated, additional security personnel likely would have been assigned to the event.
Investigators also found that the Secret Service failed to ensure that the American Glass Research facility—where Crooks positioned himself to fire at the stage—was properly secured by state and local law enforcement. Despite identifying the rooftop as a potential vulnerability, the agency also failed to use available resources to block its line of sight to Trump’s speaking position.
The inspector general issued seven recommendations aimed at strengthening Secret Service protective operations. According to the report, the agency agreed with all seven recommendations and has already implemented some of the proposed changes while continuing work on the remaining reforms.
The Butler attack has remained one of the most scrutinized security failures in modern Secret Service history, prompting multiple congressional investigations and internal reviews into how an armed gunman was able to get within firing range of a presidential candidate.

Butler was a Deep State failure to assassinate President Trump.
To me it looks like the mistakes were not accidental. Too many coincidences.
That’s more warnings than Pelosi has Dalmatians!
Get them all in separate rooms and question them until somebody cracks. Play good cop bad cop. Make noises in the room next door of chairs breaking and people screaming. Then waterboard them.

The biggest failure might have been not putting a man on the rooftop that was the single best shooting platform.
No one would have climbed up on the rooftop if there had been just one person up there.
Obviously they are not the best and the brightest.
