I think the use of “anonymous sources” should be illegal.
Publish verified facts, with people willing to stand behind those facts.

Publications that release information (even in good faith) which is later found to be untrue should be liable for judgments in the tens of millions of dollars.

Publish the truth. Why is that so hard?

I think the use of “anonymous sources” should be illegal.
 The argument has always been that if you do that people won't come forward with any information that could get them in trouble. I do agree that something needs to be put in place where all sources should be registered with an unbiased third party and penalties should be there if the source turns out not to be telling the truth. How you could pull that off I don't have a clue. 
 Like it or not, the current state of "journalism" has proven they can't be trusted to do the job correctly or honestly so some changes are going to need to be made.
