There is a "real possibility" that a regional crisis with either China or Russia could escalate to nuclear war, the head of U.S. Strategic Command is warning in a call for federal and military leaders to change their approach to nuclear deterrence.
"There is a real possibility that a regional crisis with Russia or China could escalate quickly to a conflict involving nuclear weapons, if they perceived a conventional loss would threaten the regime or state," STRATCOM Commander Adm. Charles Richard wrote in the February issue of "Proceedings," the monthly magazine for the U.S. Naval Institute. "Consequently, the U.S. military must shift its principal assumption from 'nuclear employment is not possible' to 'nuclear employment is a very real possibility,' and act to meet and deter that reality."
He also warned that China and Russia are beginning to "aggressively challenge international norms and global peace using instruments of power and threats of force in ways not seen since the height of the Cold War. He also cited investment in advanced arms, such as nuclear weapons, as well as a rise in cyberattacks and threats in space.

Russia should be a natural ally of ours when it comes to China. Explaining why it’s not is above my pay grade.
Almost anything is possible, but nuclear war would not be in the best interests of the United States, China or Russia. So it would be unlikely to happen.
OTOH, N. Korea or Iran doing something stupid is more likely. (But let's spend gazillions of dollars to prevent something that's not at all likely to happen, instead.)
There is no scenario where one sides pulls the nuclear trigger first. Suicide is never an option. Eventually the rulers have to come up from the bunker. They can’t live their forever...

I disagree. If any side could win a nuclear war it would be the side that strikes first. Thus, there would always be significant pressure to push the button first in any serious conflict.

I thought I left the fears of nuclear war behind in the mid- 90’s. Just what I need now.
I’m 1/4 mile from a major Army/Air Force joint base.
In other words...Ground Zero.
When tactical nuclear weapons can be cleaned up to the point where they are really just big bombs (little to no fallout risk), we could see the unwritten “moral” ban on “any” nuclear weapon use eliminated. In which case, yeah, we could see nukes used in war and battles, just as other weapons and bombs are currently used.
And then pandora’s box is opened and things escelate.

So the American Marxists certainly THINK they can win the next one, but they now hold fewer than one fourth of the cards and aren't the only player at the table, unlike the last time when they held ALL the cards.
