As I understand it accretion disks are made mostly of matter which has “radiated” out of a spinning black hole. The “inner” event horizon grows at the equator, and becomes an elipsoid. Where the inner horizon crosses the outer horizon particles form randomly because of heat radiation. The physics of heat radiation in relativistic environments changes to allow matter to radiate. Most of the resulting particles fall back into the singularity, but the particles traveling outward at the speed of light stay in our universe.

Black holes are regions of infinite density, known as a singularity. All models are wrong; some are useful. While infinities are common in the abstract world of math, no infinities actually exist in the real world. When a math model hits an infinity, it has gone off the rails and ceased being useful.
As a black hole grows in size, its density decreases. There are super massive black holes with a mass density less than water.

Photons are particles that have relativistic mass.

I though the speed of light was a constant and was as fast as any thing could travel. Mathematically you can travel faster than the speed of light. Einsteins equations permit it. It is just that you can't travel AT the speed of light , and there is the rub.
