There are a number of ways. And some of it has nothing to do with a counting person until the ballot is rejected by a machine that can count up to 32k an hour.
A ballot may be spoilt in a number of ways, including filling out the ballot in a manner that is incompatible with the voting system being used like marking more choices than permitted (overvoting).
It has been noted that if a ballot was folded and run through the machines for count, it would register in the counter as a second vote for the part that was creased. That vote was then rejected by the counting machine and sent back to the people running it. It was then scrutinized by the counters and either rejected or entered as a vote for the actual marking and not the crease. In other words, the people that oversaw the machine were actually doing the voting and not the counting machine. Any mistakes, or just outright illegalities, would register a vote incorrectly for the wrong person. The dominion machines are tied into this problem. So how many votes were actually done incorrectly as the votes were folded to be put in the jackets in some states? No way of knowing. But with the election results in many states being a few thousand after a couple of million, it is an easy way to rig an election. Needs to be investigated.

There are many problems that need to be addressed with the actual count besides the graft used to change or reject existing votes. And a rejected ballot can further be rejected by choice of the employee doing the examination of the machine reject.

Someone might fold a fraudulent ballot if they thought it was supposed to look like a mail ballot, bit then because of timing issues they needed to be fed into the machine as in person ballots. I’m pretty sure observers have been told what to look for.
