The above is just part of it - 25% indoor occupancy and the owner or waitress comes around with a notebook asking you to print your name and provide a phone number. Not yet required at the big box stores or super markets.

We had a wedding in July (I think the rules are less strict now), and the caterer said we had to provide them with a list of all the names and addresses for those who attended (for them to be able to turn over to the county health dept, I suppose). We didn’t provide the list, and they didn’t ask us for it again, and no one who attended came down with Covid-19 within the 2-3 weeks following the event.
