Most recently with the Littoral Combat Ship. Government got exactly what it asked for, and it’s useless in the spec the government ordered it in.

And in the "government designed single source" category (which the LCS isn't single sourced), the Army Combat Uniform and the Universal Camouflage Pattern, which is two lies in one - it's not universal and it's not camouflage unless you are trying to blend in to grandma's couch or some hotel drapes.
