COVID-19 Deaths: Italy, 1 in 5,789 People; United States, 1 in 157,499

(CNSNews.com) – From Hillary Clinton (“He did promise ‘America First’”) to MSNBC’s Chris Hayes (“We are quite literally doing a worse job of containing the virus than any other country on earth”), critics are trashing President Trump over the fact the U.S. is now reporting more confirmed COVID-19 cases than any other country.

But numbers of confirmed cases are a function of testing. As testing in the U.S. has ramped up, it was only a matter of time before that testing would detect sizeable numbers of cases moving through the American population.

Some of the world’s most populous nations, Pakistan and Indonesia among them – have carried out very little testing for countries with populations of 233 million and 267 million, respectively. India (pop. 1.3 billion) and Nigeria (pop. 214 million) are even further behind.

In the United States (pop. 332 million) more than 850,000 tests had been carried out as of early Monday (more than 710,000 of them negative), according to the COVID Tracking Project.

Of the 12 countries reporting the highest numbers of deaths – those seven plus the Netherlands, Germany, Belgium, Switzerland, and South Korea – Italy leads not just in overall numbers, but also has the highest number of deaths in proportion to its population, at 0.172 percent. Put another way, one in 5,789 Italians has died of the disease caused by the novel coronavirus.

The next highest per capita COVID-19 death toll is reported from Spain, where one in 7,571 people have died.

In distant third place is the Netherlands (one in 22,412), followed by France (one in 26,035), Belgium (one in 27,194), Switzerland (one in 28,013), Iran (one in 32,167), and Britain (one in 53,551).

In ninth place, considerably lower down is the United States (one in 157,499), followed by Germany (one in 163,591

To all medical professionals around the world:

My name is Dr. Zev Zelenko and I practice medicine in Monroe, NY. For the last 16 years, I have cared for approximately 75% of the adult population of Kiryas Joel, which is a very close knit community of approximately 35,000 people in which the infection spread rapidly and unchecked prior to the imposition of social distancing.

As of today my team has tested approximately 200 people from this community for Covid-19, and 65% of the results have been positive. If extrapolated to the entire community, that means more than 20,000 people are infected at the present time. Of this group, I estimate that there are 1500 patients who are in the high-risk category (i.e. >60, immunocompromised, comorbidities, etc).

Given the urgency of the situation, I developed the following treatment protocol in the pre-hospital setting and have seen only positive results:

1. Any patient with shortness of breath regardless of age is treated.

2. Any patient in the high-risk category even with just mild symptoms is treated.

3. Young, healthy and low risk patients even with symptoms are not treated (unless their circumstances change and they fall into category 1 or 2).

My out-patient treatment regimen is as follows:

1. Hydroxychloroquine 200mg twice a day for 5 days

2. Azithromycin 500mg once a day for 5 days

3. Zinc sulfate 220mg once a day for 5 days

The rationale for my treatment plan is as follows. I combined the data available from China and South Korea with the recent study published from France (sites available on request). We know that hydroxychloroquine helps Zinc enter the cell. We know that Zinc slows viral replication within the cell. Regarding the use of azithromycin, I postulate it prevents secondary bacterial infections. These three drugs are well known and usually well tolerated, hence the risk to the patient is low.

Since last Thursday, my team has treated approximately 350 patients in Kiryas Joel and another 150 patients in other areas of New York with the above regimen.

Of this group and the information provided to me by affiliated medical teams, we have had ZERO deaths, ZERO hospitalizations, and ZERO intubations. In addition, I have not heard of any negative side effects other than approximately 10% of patients with temporary nausea and diarrhea.

In sum, my urgent recommendation is to initiate treatment in the outpatient setting as soon as possible in accordance with the above. Based on my direct experience, it prevents acute respiratory distress syndrome (ARDS), prevents the need for hospitalization and saves lives.

We were never “behind Italy.” Italy - like NYC - was exceptions rather than the rule. They got hit by the perfect storm: older population plus very high travel between China and Italy. When they got hit, they got hit hard.

Factors I see in the difference:

Older population
Higher incidence of smoking
Multigenerational housing
Higher density of population
Lower quality healthcare
Higher incidence of Chinese populations across Northern areas
Slower acceptance of “social distancing”
Celebrating “Hug a Chinese Day” early in pandemic

In Steven Chowders latest show they reference an observation from Italy...1 in 3 deaths are people that would have predictably died within the year from underlying disease(s).

fyi: my lab contacts are now doing 1000+ runs, many times a day with less than a day turnaround. also many more types of tests coming online.

and that’s just one private lab, admittedly a top lab, but this chinese virus may be able to run, but it can’t hide in America anymore.

The more relevant number, from my perspective, is the number of deaths to the number of people infected. Unfortunately, we will never know how many people were infected, as I’ve seen estimates that from 25 to 85% of people will have zero symptoms, and will never be tested. And even those with mild symptoms may never be tested.

At the moment, the US “fatality rate”, based solely on the number of confirmed positive tests, is 1.8% (2805/151,944). The fatality rate in 11.4%. That’s about 6 times higher than the rate in the US. I suspect those percentages are inflated for both countries, due to the number of people who were infected and didn’t show symptoms or who will never be tested.

If you look at actual deaths per 1 million population, the US is at 8, next to Germany at 7 per 1 million. Spain is 157 and Italy is 192. San Marino, a small principality within Italy, is the world’s highest at 737 per million, although they’ve actually only had 25 deaths.

US is 13th in a list of the top 15 countries for deaths per million population. https://www.mediterranee-infection.com/covid-19/ (scroll down). Almost all the top 10 are in western Europe. So, the US may move higher in the list in the next week or so.

However, same reference shows patients in hospital being treated with HCQ/azithromycin: 1291; deaths: 1.
