Trump didn’t shake Pences hand either. It’s not protocol to shake their hands at this point so it would not be expected or anticipated. He was already turning towards the podium when she extended her claw so he most likely didn’t even see it anyway. There are other points to this as well like Pelosi not giving Trump the traditional introduction.
Pelosi departed from normal decorum in introducing the President when he entered the chamber. She failed to say it was an honor and a privilege to introduce him. Trump didn't shake hands with her or Pence. Tearing up the speech behind his back was disgusting and petty.
He didn’t shake Pence’s hand either. He just handed them their copies of the speech and then turned to the podium - I don’t think he even saw Nancy’s hand. Pence didn’t hold out his hand, so its probably not protocol.
