The errors led to unusually severe cracking in the concrete that should have worried engineers and prompted the closure of the roadway below for safety.

About 25 years ago, I was with an engineering company that 10 years earlier had designed a pipe rack network system 10 years before that. In the collection of pipes the rack carried was an in service pure oxygen line. During removal of some out of service lines, a welder cut into the oxygen line and an explosion blew him up right good. His survivors sued every party possible for damages.
The cause of the explosion was that the area the accident was located was heavily fouled with coal dust. Combustible dust plus oxygen plus a cutting torch = big kaboom
I did a deep dive into the engineering records and these were turned over to my company's lawyers. The engineering company was released for the civil suit within a few weeks with zero liability. Why were they released? The root cause was simple to identify. Dirty exterior on the pipes obscured pipe labels and this led to owner and maintenance company failures to properly identify then eliminate the hazards prior to demolition.
As engineers, my company plainly demonstrated to the court and was summarily dismissed from the lawsuit as pipe labeling specifications for type, size and location that were in conformance with OSHA standards in effect at the time of the design. Note that the legal standard for engineering designs is to be in conformance with generally accepted engineering practice and standards. The does not say perfection.

Liability is joint and several. Hang 'em all. There are no innocents in this except the victims who relied on the due diligence and competence of the authorities and builders, reliance based on years of experience with bridges that did not fall down.
