Someone 86 years old with cancer? On top of the medications, emotional issues etc, is calling shots and making this level of decisions in the highest court in the land?

Not nearly as important as some folks would like to believe. Justice Thomas is on record saying that they could do away with 'oral arguments' completely and lose little, if anything of value. This is one reason he so rarely participates in them. As far as he's concerned, all the meat is in the briefs, and the orals are nothing but theater.
On the other hand, I do think it would be valid to ask if any drugs she might be on could affect/cloud her judgement. My late wife had cancer, and went through two rounds of chemotherapy. She often complained about 'chemo fog', which was when the drugs she was on would make her very forgetful and easily confused.
If RBG died in 2020, the GOP promised not to confirm a scotus judge in a presidential election year. That’s not in the constitution, but the explanation why McConnell didn’t hold a vote on the Obama nominee Garland. I don’t think they mentioned the circumstances of Scalia’s death.

Dude, I pulled that comparison pic from twitter for discussion. Read my note above the pic, as I noted that it wasn’t a match, IMO.
Lets just say most of us felt the proof was in the pudding, and if she wasn’t already dead she was one foot in the grave and the other on a banana peel; and we most certainly would not put it PAST the opposition to pretend she was alive when that was not the case. Not necessarily that they would try to pull it off indefinitely, but certainly to drop as a bomb on a day politically favorable to them. You can keep a body at 14 degrees for a long long time.
