A Trader Joes employee, who died during a hostage situation over the weekend, was killed by LAPD gunfire, Chief Michael Moore said Tuesday at a news conference.
Ballistics tests show that a bullet fired from a Los Angeles police officers firearm struck and killed Melyda Corado, 27, during a shootout with the alleged hostage taker at the Silver Lake supermarket Saturday, Moore said.

Well, there you go (if I understand your post). There was no reason to shoot at that time, given that he handcuffed himself and surrounded himself to prevent being shot. So, the police shot anyhow, killing a hostage. Nothing will happen.
With affirmative action - mostly geared toward women - the 'in fear of' and 'reaching for' along with 'split second decision' became part of a complete legal exemption in shooting someone, anyone... at anytime.
Any police procedure overruled basic law. No longer did decisions to take a life need to be made 'by a reasonable and prudent' person, they simply needed to be part of 'police procedure' and solely in the mind of the officer onsite.

That reminds me that I left out ‘failure to obey’ as another reason.
Very tragic and extremely unfortunate. :-(
But let's not suggest that there may be a smidgen of potential for improvement in tactics, training, or attitudes of our heros in blue.
