I think Rand needs to differentiate what he means by "republicans" There are two distinct and horizontally opposed versions of "republican.

there is the republican voter (who is republican by the intended ideals) AND THEN THERES THE REPUBLICAN POLITICIAN ( who is republican because it was the only slot open, all the democrat voting blocks were taken).

the Republican political elites are just the same as the democrat politicians and neither elitist group has a grip on reality, or give a rats behind about what makes America Great they are just greedy gangsters, all they care about is their power and greed and how to please their lobbyist constituents, The poor masses in the streets are just useful idiots to them, healthcare is a perfect example to make that case

What made America great was a FREE an unencumbered low taxed citizen without GOVERNMENT interference. New ideas were created new jobs look at what happened in the 1920s when the republican president did just that,
