The state border restriction could be eliminated if coverage was split between drugs and care.
You might buy a care policy for your area/state and a drug plan from a company buying for a nationwide customer base.

Things like many cancers, HIV, Hep-C and auto-immune conditions take months or even years to have serious impacts. If you have depression, you have to go through step therapy using the cheaper drugs (~one month for each one, of which there are many).
PPACA policies generally have high deductibles, often over $6,000. A person with a passport (card) can get surgery in Mexico for less (and in two days or less, not 15 or 30).
The actual moral hazard is in the EMTALA, not with insurance.
There is no simple fix.
What I suggest to help:
1. federal government recommended fee schedule
2. allow three foreign doctors in for each American doctor who refuses to accept fee schedule in writing, with the foreign doctors having to agree to work for 20% less than the recommend fees (for American doctors)
3. allow Asian Indian hospitals to set up hospitals in the USA staffed with Asian Indian doctors
4. where an Asian Indian hospital exists within 10 miles, the EMTALA doesn’t apply anymore
5. require drug companies to sell a monthly supply of a each drug of theirs for 8% or less of the federal formula annual household student financial aid expected contribution or an amount less than $20.01 or lose federal exclusivity

This is exactly what I expected the question was a front loaded one and the way Trump answered it I was sure he didn’t mean “the mandate” ala the IRS.

What I proposed is the simple fix. What you propose is more government rules over the lives of physicians and people who develop life saving drugs. You have no business on this forum if you are here to dictate terms to anyone that doesn’t do as you wish.
