If a delusional psychotic went around thinking that he's George Washington, they rightly lock him up, medicate him, and try to dissuade him from his delusions rather than indulging him by giving him a pretend Continental Army to command.
But when an equally deranged grown man says he's a 6 year old girl, psychiatrists won't dare call it mental illness and institutionalize him, because that would be politically incorrect. Instead, they are forced to indulge the lunatic's delusions and let him live like a 6 year old girl.
