If I were a betting man, I'd say the odds are good that we are dealing with an engineer who was either sleeping or distracted. Hard to imagine that he could be sleeping since they just pulled out of 30'th street station minutes earlier and there were lots of signals and switches between 30th street and the crash site. Plus, it wasn't horribly late in the night and he's young.

Someone else on this thread astutely noted that the emergency brake may have been applied by someone else ... so there is clearly a lot that needs to be investigated.
I don't know why it would be so hard to accept the simplest explanation here. This is almost the exact same scenario we saw in a NYC commuter train derailment in 2013, and in that case it was determined to be a drowsy engineer.

Train engineers often work very erratic schedules. The NYC crash I referenced in my last post took place on a normal Saturday morning, if I remember correctly.
