The image below was sent to me by a reader who went for a visit to a physician, and was told that she could not be seen unless she filled out this race and ethnicity questionnaire, and put her name on the form.
She initially refused, but they insisted that it was required or she could not see the doctor. The form does indicate that it is mandatory under Section 4302 of Obamacare.
Section 4302 is explained by HHS as follows:

Section 4302 (Understanding health disparities: data collection and analysis) of the ACA focuses on the standardization, collection, analysis, and reporting of health disparities data. While data alone will not reduce disparities, it can be foundational to our efforts to understand the causes, design effective responses, and evaluate our progress.
Section 4302 requires the Secretary of DHHS to establish data collection standards for race, ethnicity, sex, primary language, and disability status. The law requires that, once established, these data collection standards be used, to the extent practicable, in all national population health surveys. In response to this statutory requirement, this implementation guidance outlines the new minimum data collection standards for race, ethnicity, sex, primary language and disability status for implementation in HHS, along with a description of the data standards development process, the rationale for each data standard, and instructions for their implementation.

Since you are whichever sex you feel like being on any given day, I assume the same is true of race.
So, change it on each visit.

The DEA is already in the know of what medications your taking.Just try to go and buy sudafed or anything that containes Sudofedrine in it and you’ll see.
When you buy the stuff your required to give them your drivers licence and the pharmacy sends that data to the DEA.They are able to check to see if your purchasing the stuff at other pharmacies as well and if your in need of more than what the DEA says you should have they refuse to sell it to you.
They are denying medications to people who actually need it and are not abusing it.Hell its easier to buy Heroin and Cocaine than it is to buy Sudafed and other Decongestants thanks to the DEA.
My wife and I have each had an appointment since Jan. 1st using our new policies, had prepped for and expected intrusive questions, but there were none.
I googled Carolinian race. Nothing. I'm from South Carolina. Maybe we have our own race now, which would make us a minority and entitled to FREE stuff.
I can also see them using people’s medical records as grounds for denying them their Second Amendment rights. As in, if you’ve ever had one prescription for valium, or taken medication for seizures, no Second Amendment rights for you. If you’ve ever had cataracts removed, no guns for you. Of course, if you’re a criminal anyway, you could have been locked up in the psych ward for threatening to kill your former boss and co-workers, and still get a gun.
Apparently, this option must have been used too much, making the governments data collection worthless because I see that this option has been done away with.
From now on, OTHER will be the new NATIVE AMERICAN. Give the government nothing.
Or India. I remember reading a recent article about a scandal with one of India’s biggest pharma companies which supplied the US. I don’t remember the details, but their products were adulterated or placebo, and they got away with it for years, even after the FDA was alerted.
Is this list a scam? La Raza is a ethnicity? why is there no listing for Latino, Castilian, or what ever Mexicans are?

I have always wanted to identify as Vulcan.
I had to fill out a form recently during a visit to a doctor, but mine was nowhere near as thorough as the one shown.
It asked for race, and it gave several choices, including “other”, which was followed by a blank, on which I wrote “human”, as I always do. It also asked for my ethnicity, so I wrote “American”. I don’t recall being asked before for my ethnicity.
your race comment, i thought that too. it is utterly ridiculous. really the ethnicity one is screwed up too.
