I was asked that question 5-6 years ago. My PCP, a friend and former customer of my now-closed Family Farm Business for 22yrs and I shoot together, monthly, so he put a ‘NO’ on the answer block.
Just lie and say NO.
According to Catholic doctrine, if the questioner has no right to a truthful answer, you don't have to answer truthfully. This falls in the same category as the question "Are you hiding any Jews in your house?" Or, a couple of centuries ago, "Are you hiding any escaped slaves in your house?"
They don't need to know in order to take care of your health. Tell them only what they have a right to know.
