A Senate committee has refused to approve a promotion to admiral for a Navy officer who, as a young fighter pilot during a training mission, deliberately shot down an Air Force plane whose flier has suffered a life of pain from his forced ejection.
The Senate Armed Services Committee took no confirmation vote on the nomination of Capt. Tiimothy W. Dorsey as the 112th Congress ended.
Because the Senate did not act, the nomination goes back to the White House. The Navy has the option of trying to resubmit his nomination during the 113t Congress, which convened Thursday.

FWIR of the documentary, the excuse given was that the expenditure rate of munitions was so rapid in Vietnam at the time, WWII-era stuff, aka "unstable". was being pressed into service.
The bomb or rocket that decided to spontaneously catch fire, eventually setting off a chain reaction of explosions, just happened to be on McCain's aircraft.
McCain escaped the escalating bonfire by climbing out over the nose and refueling probe.

Oh please. A Zuni Rocket on an F-4 across the deck from McCain's plane fired because of an electrical fault and hit the A-4 next to McCain's plane, setting it on fire. McCain saw the fire and jumped out of his plane before the bombs in the fire starting going off.
McCain has ZERO responsibility for the Forrestal fire.
McCain crashed two aircraft - one was judged to be his fault, the other was an engine failure.
He was shot down over Vietnam, once.
His plane was destroyed on the Forrestal but of course that wasn't his fault.
There was one incident where he flew too low hitting power lines in Spain, but he landed safely.
So McCain crashed ONE aircraft where it was his fault.
