How does one go from a bachelors degree to a doctorate program without earning a masters, or am I missing something here?
It's not that uncommon; there are programs to do a Ph.D. straight from a Bachelor's. I don't know much about them, but I have heard of them.

There a few things that still don’t make sense. Hopefully it will all come out in the wash.
I am just trying to collect snippets of information and place them in some sort of order so that I can try to understand what isn’t understandable.
The media doesn’t seem to be laying out any sort of emerging chronological image for us, it just seems like a bunch of puzzle pieces dumped on a table.

Many doc programs do not earn masters. Some will give a masters if you flunk out the doc but not all.
