Crony capitalism at its most basic. Create onerous regulations and then give out exemptions to allies and constituents. The only wrinkle in this case is that the demographic subsection unburdening itself of ObamaCare is the same demographic subsection that has been most pointedly exuberant in advocating health care reform, with California Democrats even passing symbolic health care laws just like the ones that their most liberal supporters are now opting out of.
On the federal level, of course, Former House speaker Nancy Pelosi was instrumental in securing ObamaCares passage. And now, for a disproportionate number of businesses in Pelosis San Francisco district, ObamaCare is not their problem, not any more.

I called my congress critter and inquired. I only talked to a low level staffer but they seemed to think it was a worthy line of inquiry and I'd encourage everybody to talk it up.
All waivers were always to end at the end of 2013.

That is exactly right. I think the most salient point is, if it is such a great bill why do companies in her district want and need a waiver?
The House needs to pass a bill and send it to the Senate eliminating the waiver provision in Obamacare. That would set the stage for everyone feeling the financial pain from Obamacare and will lead to its REPEAL by the next Congress.
