BP plc said Tuesday an internal investigation has revealed that the failure of a number of processes, systems and equipment caused the April 20 Deepwater Horizon fire and Gulf of Mexico oil spill.
The London-based oil giant said the investigation, which was launched on April 21 by the companys head of group safety and operations, has shown much of what has been already reported by the media that multiple control mechanisms in place should have prevented the accident and reduced the impact of the spill.
I understand people want a simple answer about why this happened and who is to blame, Tony Hayward, BP chief executive officer, said in a statement. The honest truth is that this is a complex accident, caused by an unprecedented combination of failures. A number of companies are involved, including BP, and it is simply too early and not up to us to say who is at fault.
BP (NYSE: BP) also said it has not reached any final conclusions.

With appropriate caveats:
BP contracted Schlumberger (SLB) to run the Cement Bond Log (CBL) test that was the final test on the plug that was skipped. The people testifying have been very coy about mentioning this, and youll see why.
SLB is an extremely highly regarded (and incredibly expensive) service company. They place a high standard on safety and train their workers to shut down unsafe operations.
SLB gets out to the Deepwater Horizon to run the CBL, and they find the well still kicking heavily, which it should not be that late in the operation. SLB orders the company man (BPs man on the scene that runs the operation) to dump kill fluid down the well and shut-in the well. The company man refuses. SLB in the very next sentence asks for a helo to take all SLB personel back to shore. The company man says there are no more helos scheduled for the rest of the week (translation: youre here to do a job, now do it). SLB gets on the horn to shore, calls SLBs corporate HQ, and gets a helo flown out there at SLBs expense and takes all SLB personel to shore.
6 hours later, the platform explodes.
Pick your jaw up off the floor now. No CBL was run after the pressure tests because the contractor high-tailed it out of there. If this story is true, the company man (who survived) should go to jail for 11 counts of negligent homicide.

What is known for certain, Schlumberger personnel left the DeepWater Horizon at about 11:30 AM. A Cement Bond Log (CBL) test was never done to determine if the cement job/plug was holding. It is very, very unusual that a CBL test wasn’t performed after the extreme expense of drilling the well to begin with.........
Can we verify that in some way .,...I’ve been looking around and haven’t seen that>...?
