A doctor's perpective of Obamacare - Healthcare delivery bankruptcy Please read

I have been a doctor for 19 years. 4 years in the Army and 15 years in private practice. I belong to a doctor owned group of approx 350 doctors in a multi-specialty practice. We employ 4000 people. In addition to being touted as one of the very best clinics in the nation (Acclaim Award winners) we have donated over a million dollars to the local city in grants, scholarships and charity. Regularly voted as top places to work by our employees. US healthcare at its very best. I am very proud of what we do and we provide tremendous care and value to our patients.

We seek to maintain a 3-5% profit margin annually. We operate in the very precarious business model of enormous volume, low margin. As any business owner knows, this is high-risk-low-margin of error model. Consequently any small changes to cash flow vectors, mandates widespread internal policy and practice corrections. Tiny changes = massive consequences.

As many people may know, Medicare and Medicaid, the current government paid 'insurer' - pays approximately 70% of the cost of care. ie its more expensive for doctors to care for these patients than we get reimbursed for. Say you are a contractor. Imagine the government mandating a significant number of your jobs whereby your out of pocket costs are ~ 30% + greater than your income. That is Medicare and Medicaid. In perspective, our group alone, year 2008 lost ~$12 million caring for our government patients. This is despite taking over 1 1/2 years to help move our fee-for-service traditional Medicare patients over to Medicare Advantage plans, which are privatized versions of Medicare that reimburse better...still not covering costs...but lessen our losses significantly.

Many people ask, why do private health insurance premiums continue to escalate? The liberals want you to believe its a combination of profiteering and waste. When in fact its due mainly to two other processes. The first is obvious: every year it costs more to care for patients and premiums are trying to keep up with this rising cost. But secondly, and less often discussed, is that every year private delivery systems lose more and more money caring for our government patients. Someone has to make up for these losses in order for your hospitals, clinics, nursing homes, pharmacies, group practices to remain solvent and profitable. Every year these delivery systems open their books to the private healthcare insurers - and the insurers must - they must in order for the entire system at large to stay functional, increase the amount they pay out to cover these losses. If they dont, both the delivery systems and the insurers die. So to keep the boat afloat, the payouts by private insurers MUST increase to subsidize the ever increasing losses doctors incur by taking care of our government patients. So, in a way, you could say that your increasing premiums are a tax that you are paying to cover the losses that are Medicare and Medicaid. It's a clear and inarguable private subsidation of government cost. Enough said on that.

So to really feel the consequence and full impact of Obamacare, one must simply see the economic dominos. Most people can see how this bill will rapidly reduce private insurance plans and rapidly expand government plan patients. And take whatever number that is being reported, and multiply that by 3. That has been the experience in both Mass and Hawaii. Both government plans were overwhelmed with the enrollees as they significantly underestimated the government migration.

Ok, so now- how can anyone not see the obvious outcome? Government patients = significant loss of profitability. Initally the private insurers will do their best to continue to subsidize this loss, and there will be a huge escalation of premiums. But within a few months this will be unsustainable. Its a cycle that cannot be stopped. Higher premiums = higher recidivism to governmnet plans = higher premiums etc. Within months, every single hospital, every single doctor office, clinic, nursing home, pharmacy - every delivery system reliant on private insurers will no longer be profitable. ie they will go bankrupt. These will most certainly be the headlines to come: Hospital XYZ shockingly announces bankruptcy; Hospitals can no longer remain open; Clinics across the country file for bankruptcy; Loss of Pharmacy access shocks the Nation; Doctors going bankrupt en masse creating healthcare delivery and access to care crises; Where can you go to get care?; Loss of access reported Nationwide

Yes a crises. A crises of access due to widespread business failure. You will not be able to get care for as long as it takes for the government to devise their emergency bailout package and as long as it takes for those insufficient dollars to try and get those doors back open again. But it will be too late, and it will be too expensive. There is absolutely no way that our government can capitalize our entire healthcare system. Try as they might, only a percentage of what we have now will ultimately survive. And those that do survive will be a shell of what they once were. The conditions will be frightening, and the consequnces will be dire. The degree of disarray will be unimaginable and the underlap in access to care will be gaping.

I will not expand this discussion to predict what this means to our economy at large because I am not an economist. But anyone can be close to predicting what I am suggesting. Factors such as loss of work hours due to illnesses not treated, pressure on all the other private business models; let alone the out and out loss of enourmous capital via the bankruptcy of this entire healthcare industry can clearly be the death nail to our country and imo is a clear and present threat to our very sovereignty. This can make the housing collapse look like a speed bump. This will be massive and rapid and lethal and complete.

I am not certain why this very obvious outcome has not been openly discussed momre often - ie the rapid and massive bankruptcy of all of your health care providers and their delivery systems. But this is the inevitable outcome should this bill ever become law and inplemented.

I know it's a very late hour - but I do think the implementation is not an inevitability as multiple lawsuits may keep it on hold for a while - so public opinion will still be vital for many more months to come.

Hold your own street protest in front of your reps local offices. Or busy street corners. Try and get the local media to cover it. Hammer the Medicare/SS issue, nothing like mad grannies and gramps.

The National Republican Congressional Committee has published a target list on health care. In addition to continuing to contact the five Tennessee Democrat Congressmen, you can go http://www.votervoice.net/link/clickthrough/ext/94697.aspx to contact some of these targets. Much of the talk following Obama’s announcement has focused on how to defeat this second bill through reconciliation, but that is misleading because the first step to defeating Obamacare is not by concentrating on defeating the “fixer” bill but by defeating the Senate bill in the House when it goes to the floor for an up-or-down vote on Sunday March 21st.

Just hang on and when the recording is over, you will get the Capitol operator. Just ask for your Rep or Senator’s office. Then you will either talk to an aid or have the chance to leave a message for him/her to vote NO on the health care bill.

When you use the second number and the Capitol operator comes on, just ask for your Rep or Senator’s office. Every time I use this number I get the Rep or Senator’s answer machine, so it may be set up that way all the time...to go to their answer machine. Either way you can leave a message to vote NO on the health care bill!

We need to use these toll free numbers that have been set up for the health care/ BO supporters and illegals to use! After all they are FREE!

Then you will either talk to an aid or have the chance to leave a message for him/her to vote NO on the health care bill.

When you use the second number and the Capitol operator comes on, just ask for your Rep or Senator’s office. Every time I use this number I get the Rep or Senator’s answer machine, so it may be set up that way all the time...to go to their answer machine. Either way you can leave a message to vote NO on the health care bill!

We need to use these toll free numbers that have been set up for the health care/ BO supporters and illegals to use! After all they are FREE!

It will destroy private insurance companies because the fine for NOT buying private insurance will be less than the cost of the insurance. Young people will chose to pay the fine rather than purchase insurance since they cannot be denied coverage for a pre-existing condition. They can wait until they get sick to purchase insurance.

The fatal flaw for the insurance companies is that they can not survive if they are expected to cover pre-existing conditions without the pool of the insured being increased by bringing in young, healthy individuals. As explained above, there is simply no incentive for those people to enter the system until they actually get sick.

No, I sound like someone looking for a sober explanation and not hyperbole and innuendo. When I argue healthcare policy and economics with people, I want to know the details, not some emotional blather to throw at them.

And if your siblings are making less than 133% of the poverty line as I mentioned, then they are eligible for Medicaid via the vehicle that your state offers and if they aren’t on it, that is their fault. That’s what I meant by the pooorer are already on medicaid.

I am having a hard time coming up with comebacks when ppl say this program is identical to what was proposed by Dole and the Republicans as the alternative to HillaryCare in 1993, identical to RomneyCare in Mass., and more conservative than the plans proposed by Nixon and even Eisenhower. Once the public option was dropped, A lot of the conservative points of contention went right out the window.

So when I ask specific questions, I’m shilling for no one, I’m digging for information.

That does make sense, the fines are pretty low around $950 or so the first year and slight increases each year after. I agree, that is not enough of an incentive to get people fully into the risk pool if the premiums are sky high.

But I haven’t seen any cogent projections of how forcing insurers to cease various denials will affect premiums in light of at least 15 million new members added to the overall risk pool (which is a VERY conservative estimate, say 50%) actually sign up under the mandate. Have you seen the subsidy rates for the various income brackets? They are pretty hefty subsidies.

But you are right, IF the premiums skyrocket and the subsidies cannot keep up, the whole plan goes right down the toilet and all we have is monetary confiscation at the point of a barrel.

Thanks for taking the time to expand on the issue. So it is as I suspected, this is a backdoor route to single payer based on possible future laws, not current ones.

They did kick around expanding Medicare to age 55 just a few months ago but that didn’t last more than two days. Which kind of begs the question. If we have the most liberal house, senate and president that we have seen, IMO, in probably 70 years and they can’t pass a public option, how can government run insurance be expanded after this current group inevitably loses power in possibly 2010 and most definitely by 2012? Is your argument that the public will be so decimated by this that they will force even a Republican administration to expand government insurance? I’m not ruling that out, Johnson did the same with Medicare and Medicaid and it has become one of the third rails even with even conservative politicians, evidenced by Gingrinch’s results in 1995.

My hunch is that we will end up like Germany who has a system quite similar to the current proposals, a mix of a government subsidized private market and a public pool (roughly 50% of the market) which is similar to our 50% public that consists of medicare, medicaid, VA, SCHIP, etc. It hasn’t crashed their system yet and doctors aren’t fleeing Germany but I will grant you they have the highest health care costs as a percentage of GDP in Europe and some of the lowest health outcomes in Europe comparatively (though they still beat us in outcomes and cost as a % of GDP).

But we are a larger and more diverse country with a totally different social and societal dynamic so perhaps it will all come crashing down and force a UK or Canadian style system of actual socialized medicine. Or at least like most Eoropean countries with actual 100% socialized insurance. Once that occurs, then I agree, the United States as we know it will have been lost.

I am a pretty busy person and I do get into and follow politics VERY closely when major events occur such as elections and major legislation. When the political environment is dry, I admit I do not follow political moves very closely.

That being said, I want you to go through my history and read carefully my posts. I look for data, evidence, actual legislation, historical precedents, surveys and other statistical data, etc. I hate hyperbole and innuendo more than anything in this world which is why I detest most politicians. But I do enjoy getting into and understanding actual policy which requires a nuanced eye toward details.

Would I align myself with some Tea Parties, Sarah Palin, Rush Limbaugh, Glenn Beck? ABSOLUTELY NOT!

Would I align myself with the writings and thoughts of Nixon, Eisenhower, George HW Bush, David Frum, William F. Buckley, Newt Gingrich and even sometimes David Brooks? Yes, that is the level of Republican I am. I like those types of Republicans because I respect the hell out of their analytical and critical thinking skills, their use of data and their derision of emotion as a basis of argument.

So before you start calling me a troll because I ask questions, you need to start throwing some data around that can be confirmed or refuted. Republicans regain power not by screaming silliness like I see at some rallies but with cogent plans on a return like the Contract with America in 1994.

You are correct that each state has the ability to define circumstances of eligibility beyond poverty so perhaps a grass roots effort to get your state to expand the definitions of those circumstances are in order?

But are you not making the argument that there needs to be an avenue for a person to enter the private health insurance market via subsidies if they are unemployed or at the poverty line? I know you will continue to call me a troll for asking that question but if you follow the logic carefully, that is exactly what you just did vis a vis the emotion evoked with regard to the circumstances of your son and nephews.

I did go through your posting history, what little bit you've done since 2004. And your posts leave one to ask, "How much do the deem-ocrats pay you, or do you just argue their line for free?"

NO. I am making the argument that you are a troll. Maybe a paid deem-ocrat shill, I don't know, but a troll none the less.

Forcing people to buy insurance is forcing them to buy a service. And that it is against the U.S. Constitution. Now I know you deem-ocrats don't think much of our Constitution, other then wanting to use it as toilet paper to wipe your oversize loads with. But still, it is only by the rule of law that this country, any country can stand. With out the rule of law you have nothing but tyranny.

I also remember the worst president in the history of this country. And I remember how life was with carters high tax's and liberal socialist policies.

I remember all the business's closed up and stood like empty shells in our small town leaving only the gas stations and bars open. Our once happy little town was like a ghost town, empty of people on the streets because no one had any money to go anywhere, do anything.

And I remember going hungry. Giving what I had to my kids, but still they cried hungry because we didn't have more to eat. I remember spending one whole winter with no heat. I remember both my ex husband and I working full time jobs and still we couldn't afford coffee, sugar, or meat, except for chicken backs and necks. I even burned coil oil lamps to cut down on our electricity bills.

And I know what this health care bill is going to do to this country with all the tax's they added, I lived that tax hell once already under the liberal carter. And I don't want to go there ever again!

And regarding your other post “what little bit you’ve done since 2004”... Are you being malicious, obtuse or both? Have you seen the full history, length, depth and inquisitiveness of my posts? It’s not like I have posted 9 or 10 little one liners since 2004.

Dig deeper, go back more than one or two pages of my history, read and try to understand my lines of logic in these and you will see how silly you sound. I have over 500 posts most of which are hundreds and hundreds of words and multiple paragraphs in length debating very nuanced and complex topics.

Just because I think the likes of Glenn Beck and Sarah Palin are bad for the Republican party and people like Newt Gingrich and the writings of David Frum, the policies of Richard Nixon and the theses of William Buckley are good for the Republican party does not make me a democrat troll.

We either keep the tent open or the party splits and we lose it all. Scott Brown is a case in point. A RINO who will vote against the party platform perhaps 50% of the time? Yes. Better than a Ted Kennedy successor, ABSOLUTELY!

You, it seems, are looking to diminish the square footage of the tent and that is bad for both the party and the country.
