The simple reading of this form does not reference the issue of "Have you ever practiced law under another name?", it seems pretty clear the question is "Have you ever been KNOWN by any other name?". I think he was known as Barry Soetoro, Barry Dunham, and maybe other names in is checkered lifetime. What am I missing?

What’s so brazen is the specificity of the lie:

“I was in the stands and from a distance at a Chicago Bears Football Game.”

Obama would have been better off to say,
“I don’t recall, I meet so many people. I may have.”

But this is a old tactic.
Be specific in your lie and people are supposed to swallow it hook, line, and detail.
