Almost certainly a holstering accident, caused by negligence and/or poor training.
Before the Glock haters chime in, this can happen with ANY firearm - even an old double action revolver, as the NYPD proved for decades.

Maybe I’m a little dense but shouldn’t you *remove* any bullets before attempting to clean a gun? I’ve never understood why there’s a bullet in the chamber to go off in the first place.
Attempting to clean a gun while a bullet is in the chamber is like attempting auto repairs while the car is running, dontcha think?

Thanks to the NYPD, the “accidental discharge” numbers are still higher for the Smith and Wesson DA revolver than the Glock autopistol.

Now after reading the article again, I realize it wasn’t her fault. The gun “went off” while she was cleaning it. /s
I love the “writers” who put these stories together. None of my guns ever “went off”. Where did it go? Did they find it? Did it come home on its own?

Yup, you’re supposed to check and double check (then check again) that the piece is unloaded prior to disassembly. Especially since many autopistols require you to pull the trigger to disassemble the unit.

There is no such thing as an 'accidental' discharge if the weapon is under control, it is call a 'negligent' discharge.
The only thing NYPD uses the revolver for, is to lean on while they finish their dounut.
