He only makes the ‘flipping off’ gesture one time. As far as I know, there are no other examples of him doing this before or after.

He gave the message he intended, his supporters responded exactly as he expected, and he intentionally kept a straight face throughout, to maintain plausible deniability.

If he didn't intend to flip the bird, or hadn't realized it, he would look shocked or confused. He doesn't. He consciously maintains a straight face.

Just like with the “lipstick on a pig” comment.
