My sister and my niece-in-law both have physical conditions that made them lose a great deal of weight which drew plenty of "gee, you look great" comments. Both are generally okay but it's bad form to tell them how good they look.

Both my Mom and Grandmother had chemo and radiation for cancer. Both lost their hair and when it came back it was no longer gray, but instead a soft brown (neither had brown hair before). I don't recall them losing their wrinkles, however.
