I magnified a copy in PSP but can't see any difference in pixel size between the hat and those around the hat. The thing is, there's no reason for faking more smoke or weird smoke. Except exaggeration. Maybe that's enough for Adnan.
The perspective of the two images is very different. The background image is much more of an upward vertical view.
I blew it up in my image manipulation software and what I see is consistant with the type of artifacting you get when compressing an imaging using something like the JPEG (.jpg) standard. The fine pixels are artifacts of the compression, not of photoshopping.
