I magnified a copy in PSP but can't see any difference in pixel size between the hat and those around the hat. The thing is, there's no reason for faking more smoke or weird smoke. Except exaggeration. Maybe that's enough for Adnan.

Another possible reason would be to cover up something that the photographer doesn't want you to see.
