The comparison in the article is weapon to weapon. One revolver in the hands of a five-year-old would kill one person. One nuclear bomb in the hands of Iran, delivered by NK, or in the hands of Al Qaeda, would kill a million people, give or take.
All out thermonuclear war could kill billions as you note. But Iran, NK and the terrorists won't get an arsenal like the USSR had.
